US Visa Examination

Examinations for US visa applicants (Green card examinations)

Fees

Examination fee for adults: $170.00
Examination fee for children (up to and including 15 years): $90

These must be paid in cash or by cheque on the day of the examination. In addition, you need to have a chest X-ray and blood tests to check for HIV, Syphilis & Varicella. You will be sent to two nearby organisations for these. These are Steg Radiology and Dorevitch Pathology, which are located on separate floors at 20 Collins Street, Melbourne.
Fee for X-ray: $75
Fee for blood tests: $60.35
You also need to pay these fees on the day. These organisations do take bank cards and you can also pay by cheque or cash: however, you need to supply the exact money as they cannot give change.

Children do not need to have chest X-rays or blood tests

Total cost for an adult = $305.35

Note:
  • These fees are current as of the date of this website. When you ring to make an appointment you should check to see if they have changed.
  • These fees are not claimable under Medicare.
What to bring with you
  • All the forms the American consulate has sent you. Do not fill out any of these yourself
  • The form called Medical Instructions. This has our details on the back
  • Your passport
  • One passport-sized photograph
  • Your case number, if this information has been sent to you
  • Any records you possess of any vaccinations you have had
  • A cheque book or cash to pay for the medical examination
What to expect

Before the examination
You will be asked for your documents, your passport and the photograph, and the receptionist will indicate which part of the forms you will need to fill in. She will fill in a patient card and part of the forms. As this takes about 10 minutes, it is wise to arrive at least 10 minutes early for your appointment.

The examination
This consists of a general medical checkup of your heart, lungs, blood pressure, reflexes, eyes and external genitalia. Please discuss with the doctor if you have any concerns about this at the start of the consultation. You will be given two referral forms: one for the X-ray and one for the blood tests.

After the examination
Go to 20 Collins Street for your chest X-ray and blood tests. This is about four blocks away or a short tram ride. You must make sure that the technicians at each place sign the Medical Instructions form (the one with our address on it) and that you keep this to hand in at your interview in Sydney.
You may also need to go to your usual doctor to have your vaccinations updated.
Allow approximately two hours for this process. Note that if you arrive late to your appointment it will probably have to be rescheduled.
It takes three to five working days for the X-ray film and the blood test results to be delivered to the surgery. Ring after this time to arrange a time to collect your documents and the X-ray from the surgery. If it is difficult for you to collect the documents in person you may arrange for someone else to do so (they will need to supply a letter of authorization from you and their own identification, such as a drivers licence or passport) or for an additional fee to cover expenses you may arrange to have it posted.

Note
  • If you go to a different radiology place for your X-ray or a different pathology place for your blood tests from the ones we normally send people to you will need a different referral form and the process may take longer.
  • Dr Ralph Poppenbeek and Associates take no responsibility for any loss due to the documents being posted or to failure to collect the documents.
  • This examination is valid only for a year from the date of the examination.
